Surgeons and hospital staff can refer an eligible TAC client to a rehab at home program.

Referrals can be made to the providers below, which are registered with the TAC to deliver rehab at home to our clients.

Healthscope

Healthscope's rehab at home program is based out of the Victorian Rehabilitation Centre, Dorset Rehabilitation Centre and John Fawkner Private Hospital. Their team of health professionals helps patients achieve their rehab goals following surgery, illness or injury in the comfort and privacy of their own home. The program is delivered across metro Melbourne.

Epworth HealthCare

Epworth's program supports patients discharged directly to their home, allowing them to return home from hospital sooner with home-based support in place of a rehabilitation hospital stay. This gets them back into familiar surroundings and on the road to independence sooner.
