Injury Support Program
The TAC’s Injury Support Program applies value-based healthcare principles to identify and address barriers to a client’s recovery. It provides proactive, evidence-based injury management care coordination through our external partner Honeysuckle Health.
Honeysuckle Health’s team of experienced Injury Nurse Coordinators work with eligible TAC clients to link them with high value care and supports in the local community. The Injury Support Program is designed to complement the usual treatment and support a TAC client would receive.
The program lasts up to four months. The TAC pays Honeysuckle Health to deliver the program at no cost to the client. Participation is voluntary – the client can decide if the program is right for them.
How the Injury Support Program works
Working with the client, the Injury Nurse Coordinator develops a plan to help them:
- Coordinate medical care and appointments
- Connect with additional resources
- Obtain clinical advice and support
- Build capability to self-navigate the health system
The Injury Nurse Coordinator may ask the client’s GP or other health professionals for their advice on the plan.
The Injury Nurse Coordinator will then stay in touch by phone or email to help the client:
- Organise and attend healthcare appointments
- Work with the GP to get referrals to appropriate services
- Understand and manage their recovery, symptoms and medications
- Access resources that are right for them
Your role as health provider
An Injury Nurse Coordinator from Honeysuckle Health may contact you if your patient is participating in the Injury Support Program. During the program, the TAC can pay for the reasonable treatment below if related to the person’s accident injuries and coordinated by Honeysuckle Health:
- Medical consults
- Allied health treatment
- Medical diagnostics
- Up to 6 sessions for mental health supports (any further treatment requires a Mental Health Treatment Plan)
You should continue to submit an Allied Health Treatment and Recovery Plan but you do not need separate approval from the TAC to provide these services during the Injury Support Program. Please speak to your patient or Honeysuckle Health to confirm the beginning and end dates of the program. This is outlined in the client’s Injury Support Program approval letter, which they can supply a copy of to you.
Payment for treatments you deliver
Please invoice the TAC for the treatment you deliver during the Injury Support Program. In most cases, the TAC will pay you directly at the rate listed in our fee schedules. If you charge more than this rate, please speak with your patient about any gap payments.
If your patient pays for the approved treatment or service in full, we will reimburse them at the TAC rate. Please ask them to scan or take a photo of their receipt and send it to us through the myTAC portal or app.
